DURING YOUR STAY
- Meals — Each resident is evaluated by thein-house, full time Registered Dietician(RD) who closely monitors the nutritionalneeds of all residents., and an individualized nutritional pan is developed to assist the resident in healing. Meals are planned by the RD and dietary manager who meets with each resident to discuss their needs, likes and dislikes. Specialized diets are also available
- Telephones — Local telephone services is available in each room or you can use your cellphone.
- Television — Cable television is available in each room. Your TV can be operated from your bed with a handheld control unit.
- Salon and Spa Services — a barber/beautician is available by appointment at the center. You will be able to schedule an appointment by contacting the unit secretary. (Payment will be taken at time of service)
- Money and Valuables — You may wish to keep a small amount of money for a newspaper, stamps, or for vending machines. Please leave valuables at home
- Mail & gifts — Delivered daily to patient rooms. Mail received after your discharge will be forwarded to the address provided on your admission form.

DIFFERENCE BETWEEN SKILLED NURSING & REHABILITATION CENTER AND HOSPITAL:
- You have the option to wear your own regular and comfortable clothes
- You may use your own personal care items
- An intake assessment will be taken upon arrival to the center
- You will be followed by a physician who will be in contact with your specialist as needed. The physician will visit you within 2-days of your admission.
- 24/7 visitation policy.
- Wireless internet service is available if you would like to bring a laptop/tablet.
- You may enjoy some fresh air in our courtyard.


OTHER REQUIRED INFORMATION
- Patient’s Medicare, Medicaid, Manage Care, Social Security, and Insurance Cards- including prescription drug cards, if applicable
- Patient Proof of Citizenship or Residency (e.g., U.S. Passport, or Birth Certificate and State Issued Photo ID). Non-Citizens must present a temporary or permanent resident card with photo
- If the patient has a representative for Financial Decisions, Supporting documents such as Power of Attorney for Financial Decisions & Health Care, Guardianship, Surrogate, or other
- In the event you have applied for State Medical Assistance (Medicaid), a copy of the application.
- Please bring a check/ credit card if paying privately.
